I'm in need of advice and if there is any local support groups in my area. I need to keep t...

I'm in need of advice and if there is any local support groups in my area.
I need to keep the city and certain details out of the public because if my ex finds out it will cause serious issues for my kids. If someone proof reading this knows where a support group is that info would be best emailed to me. The public part I will write below as I need to see if there is any holes and dangers I should be aware of.

Please keep in mind my end goal is that my children get to have a healthy relationship with their mother.

My children (11yr and 9yr) asked me last summer why things are the way they are, which I gave them the stereotypical response. They asked if we could do a 50/50 where they stay for 2 weeks at a time with us, I said I would try but would probably involve going to court if you two want to go through that. They said yes. As time passed and the abuse/neglect got worse they came to me the weekend before Halloween saying they didn't want to do the 50/50 but live with me full-time until mommy gets help and treats them better, and then they want to do 50/50.

We have not been to court for anything so my plans were to file for custody with my address as their residency with a motion for access to speed up the process of them seeing their mother. They don't trust their mother to let them come back after I have served her and I don't blame them since they're not blind when she kept them from me when she was mad. I plan to file for a OCL and hope to get my children the best life they deserve. My children want me to have full control over their lives so if they're mother stops receiving help and resorts back to her old behaviors they can just go back to visiting on the weekends. What forms do I need and what's the best route for this result? Do I bring up her mental health? Can I file for divorce without doing a child agreement as I'm scared she'll make my life as miserable as possible through the splitting of assets.

Thank you for your help and hopefully she can't figure out who this is.
